Transfer Denial Form

Required

If your transfer request has been denied, you may request that the Superintendent and/or their designee reconsider this decision. In order to request reconsideration of a denied transfer you must:

Within ten working days of receiving your letter, notify the Putnam City School District via this form that you want the Superintendent and/or designee to review the denial of your transfer request. 

Provide as much detail as possible regarding your reconsideration.

If you have any additional supporting documentation you would like to submit, you may upload them at the end of this form. You will receive an email confirmation upon successful submission of the form.

You will be notified via email and/or mail of the  decision.
