As an employee of Putnam City Schools, one benefit you receive at no cost to you is disability income insurance. This insurance helps provide income when a covered accident or sickness keeps you away from work for an extended period of time. While all employees have this insurance, many know little about it. Below are basic facts.

• All district employees receive this benefit.
• The coverage the district provides is 35 percent of each employee’s base salary.
• Coverage begins on the first day of hospitalization or on the 31st day due to a covered accident or sickness. A second plan is also available that begins on the first day of hospitalization or on the 8th day due to a covered accident or sickness. For more information about this plan, please contact the American Fidelity representative.
• The plan will pay an employee $50 per doctor visit for Sickness or $100 per accident if: you receive treatment by a physician, a claim for no other benefit is made under the policy, and the expense is not for routine medical exam, dental or eye care. No more than one benefit will be paid for the same or related condition unless separated by 14 days. This benefit cannot exceed four times per year.
• The plan will replace 35 percent of your monthly base salary for total long term disability due to a covered accident or sickness to age 65. Benefits will be reduced as outlined in brochure SB-17546-0305.
• Additional long-term disability income insurance may be purchased as a part of your Section 125 Tax Sheltered Plan. You may purchase disability income insurance that may cover up to 70 percent of your salary.
• Disability income insurance does not cover an accident or sickness arising out of and in the course of any occupation for which you are entitled to Workers’ Compensation. Please review your brochure, SB-17546-0305, for all policy provisions.
• The district Human Resources Department has the forms needed to file a claim. Claim forms may also be accessed on-line at www.afadvantage.com
• American Fidelity is the district’s Section 125 plan provider. You may discuss these options with the American Fidelity representative when he/she is at your site. All decisions regarding American Fidelity’s Section 125 must be done before Jan. 1 each year to be tax sheltered for the next year.
