District Announces Free and Reduced Price Meal Guidelines Putnam City Schools has announced its guidelines for free and reduced price meals for children served under the National School Lunch Program (NSLP), the School Breakfast Program (SBP) and the After-School Snack Program. School officials have adopted the following household-size and income criteria for determining eligibility:

Children from households whose income is at or below the levels shown are eligible for free or reduced price meals. Application forms are being sent to all homes with a letter to parents or guardians.
To apply for free or reduced price meals for their children, parents or legal guardians should fill out the application and return it to the school cafeteria or the Child Nutrition office. Applications are available at each school, the district’s Child Nutrition office at 5604 NW 41st, Suite 280, and http://www.putnamcityschools.org/Parents/SchoolMeals/FreeandReducedLunchApplication.aspx
The information provided on the application is confidential and will be used for the purpose of determining eligibility and may be verified at any time during the school year by school or other program officials. Applications may be submitted at any time during the school year.
For school officials to determine eligibility, households receiving Supplemental Nutrition Assistance Program (SNAP) benefits (formerly the Food Stamp Program), or getting TANF or FDPIR benefits must list the child's name, his/her SNAP, TANF or FDPIR case number, and the signature of an adult household member.
If you do not list a SNAP, TANF or FDPIR case number for all the children you are applying for, then the application must have the children's names; the names of all household members; the amount of income each person received last month and where it came from; the signature of an adult household member; and the last four numbers of that adult's Social Security Number, or the word "none" if the adult does not have a Social Security Number.
In certain cases foster children are eligible for free and reduced price meals regardless of the child's income. If you have foster children living with you and wish to apply for such meals for them, please contact us.
Under the provisions of the guidelines, the Free and Reduced Secretary will review applications and determine eligibility.
If a parent or legal guardian is dissatisfied with the ruling of the official, the decision may be discussed with the determining official on an informal basis.
Parents or guardians wishing to make a formal appeal may make a request either orally or in writing to Jim Hooper, Executive Director of Operations, 5401 N.W. 40th St, Suite 100, Oklahoma City, OK 73122, (405) 495-5200, ext.1212, for a hearing to appeal the decision. The school's guidelines statement contains an outline of the hearing procedure.
In the operation of Child Nutrition Programs, no child will be discriminated against because of race, sex, color, national origin, age, or disability. If any member of a household believes they have been discriminated against, they should write immediately to the State Department of Education Child Nutrition Programs, 2500 N. Lincoln Boulevard, Oklahoma City, OK 73105-4599, or to the Secretary of Agriculture, Washington, D.C., 20250.
Copies of the guidelines are available for review in at all school cafeterias and at the Child Nutrition office, 5604 NW 41st, Suite 280.
